How to Surrender Unique Disability ID (UDID) Card Online

You can choose to surrender your Unique Disability ID card if you no longer need it or due to some reason (like change of disability type/expiry of card validity). The process is comparatively simple and takes just only a few steps.

The Department of Empowerment of Persons with Disabilities allows you to easily surrender your UDID card online through their official web portal at swavlambancard.gov.in.

All you need is your UDID Number, Date of Birth, and registered mobile number.

Steps to surrender Unique Disability ID (UDID) Card online

To surrender your UDID card,

Step 1: Go to official website

1. First, go to the official website of Unique Disability ID at swavlambancard.gov.in.
2. Next, click on the ‘PwD Login’ option.
3. The login page will open.

Step 2: Login to the portal

1. Next, enter the ‘Enrolment Number/UDID Number’ in the specified field.
2. Next, select the ‘Date of Birth’ from the specified field.
3. Next, enter the ‘Captcha Code’ in the specified field.
4. Next, click on the ‘Login’ button.
5. You will be logged in to the portal.

Step 3: Surrender your UDID Card

1. Next, click on the ‘Surrender Card’ option.
2. Next, enter the ‘Reason for Surrender Card’ in the specified field.
3. Next, upload a relevant document in the specified field.
4. Next, click on the ‘Generate OTP button. A OTP will be sent to your registered mobile number.
5. Next, enter the OTP in the specified field.
6. Next, click on the ‘Submit’ button.

The confirmation message of surrender of card will be displayed. You can download the update receipt by clicking on the button.
